We are writing to make you aware of an important provider change at our Commonwealth Primary Care Ridgefield location.

Dr. Preethi R. Hilton will be leaving employment with us at Commonwealth Primary Care, Inc. Her last day in the office will be Monday, April 28, 2025. We have enjoyed having her as part of Commonwealth Primary Care’s team and wish her well as she relocates outside of the Richmond area. Dr.Hilton has expressed that she greatly enjoyed the patient relationships she formed while she was here and will miss them along with the CPC team.

Due to this change, you will need to establish care with another Primary Care Physician. We would like to assist you in making this transition as smoothly as possible.
